Eon Health Enrollment

  1. 1. When can I enroll in an Eon Health plan?

The key Medicare Advantage enrollment dates are October 15 through December 7 each year. This is known as the Annual Election Period, or AEP. At this time, you can enroll in Medicare health benefits, such as a Medicare Advantage plan, for the following year. During this period, you can also change plans or enroll in a separate drug plan. Your coverage begins January 1.

However, there are other enrollment periods, like your Initial Coverage Election Period and Special Election Periods.   1. 2. How can I enroll in an Eon Health plan?

If you are currently eligible to enroll in an Eon Health plan, you can get started right away at our Enroll Now page. If you would like to have an Eon Health licensed representative meet you in person to review plan benefits and help you enroll in person, please call Eon Health at 1-888-822-7335, (TTY: 711), October 1 through February 14, seven days a week from 8 a.m. to 8 p.m. and February 15 through September 30, Monday through Friday, 8 a.m. to 8 p.m. (you can leave a voicemail on Saturday, Sunday and Holidays) .

  1. 3. What documents should I review before completing an online enrollment?

It is important to review all plan documents prior to completing an online enrollment. It is important to review the benefits and important documents to make sure Eon Health is the right plan for you.

  1. 4. Is online enrollment safe and secure?

Yes. At Eon Health, we take the safety and privacy of members and prospective members very seriously.

  1. 5. I don’t feel comfortable enrolling online. Can I enroll over the phone?

Yes. Our inside sales representatives are ready for your call. To enroll over the phone, please call Eon Health at 1-888-822-7335, (TTY: 711), October 1 through February 14, seven days a week from 8 a.m. to 8 p.m. and February 15 through September 30, Monday through Friday, 8 a.m. to 8 p.m. (you can leave a voicemail on Saturday, Sunday and Holidays).

You can also have an Eon Health representative meet you at a location of your choosing to answer any questions about our plans or questions on the enrollment form.

  1. 6. How do I send in a paper enrollment form?

If you would like to receive a packet with plan information and a paper enrollment form, please call Eon Health 1-888-822-7335, (TTY: 711), to make this request. We are open October 1 through February 14, seven days a week from 8 a.m. to 8 p.m. and February 15 through September 30, Monday through Friday, 8 a.m. to 8 p.m. (you can leave a voicemail on Saturday, Sunday and Holidays).

  1. 7. I’m having trouble completing the online enrollment form. Who should I call?

No problem! Just call Eon Health at 1-888-822-7335, (TTY: 711) to speak to one of our inside sales representative to get help completing your form.

  1. 8. How can I be sure Eon Health received my online enrollment?

You will receive a confirmation number once your online enrollment has been completed and sent to Eon Health. Once your enrollment is approved, Eon Health will send you a letter confirming your coverage.

  1. 9. When will I get my plan ID card?

You will receive your Eon Health member ID card 7 to 10 days from the time of enrollment.

  1. 10. I am enrolling at the end of the month. Will my coverage begin immediately?

In general, your coverage is effective on the first day of the month after we receive your completed application – unless you request a later effective date or you enrolled prior to your Medicare Part A or B dates. However, it can take some time for Medicare and Eon Health to process your application, so be sure to keep temporary proof of membership with you until you receive your Eon Health member ID card.

  1. 11. When will my coverage begin?

In general, your coverage is effective on the first day of the month after we receive your completed application. If you enrolled during the Annual Election Period (AEP) between October 15 and December 7, your coverage is effective on January 1.

  1. 12. If I need coverage before my member ID card arrives, what should I do?

If you need to see a doctor or fill a prescription after your coverage begins but before you receive your ID card, use a copy of your temporary proof of membership. Temporary proof of membership could be a receipt from Eon Health, a copy of your enrollment form, or a letter confirming coverage.

Eon Health has a contract with Medicare to offer HMO and PPO Plans. Eon Health also has a contract with the Georgia Medicaid program and a contract with the South Carolina Medicaid program. Enrollment in Eon Health depends on contract renewal. Eon Deluxe (HMO SNP): This plan is available to anyone who has both Medicare and received assistance from the State. Eon Silver (HMO SNP) and Eon Gold (PPO SNP): These plans are available to anyone with Medicare who has been diagnosed with Cardiovascular Disorder, Chronic Heart Failure or Diabetes. Eon Health has been approved by the National Committee for Quality Assurance (NCQA) to operate as a Special Needs Plan (SNP) until December 31, 2019 based on a review of Eon Health’s Model of Care. Benefits, formulary, pharmacy/provider network, premium and cost sharing may change on January 1 of each year. You must continue to pay your Medicare Part B premium. Premiums, co-pays/co-insurance may vary based on the level of Extra Help you receive. Please contact the plan for further details. Medicare beneficiaries may also enroll in Eon Health through the CMS Medicare Online Enrollment Center located at www.medicare.gov. The formulary, pharmacy network and provider network may change at any time. You will receive notice when necessary Eon Gold (PPO SNP) and Eon Choice (PPO),: Out of network/non-contracted providers are under no obligation to treat Eon Health members, except in emergency situations. For a decision about whether we will cover an out-of-network service, we encourage you or your provider to ask for a pre-service organization determination before you receive the service.
